Student-Run Free Clinic operates on Saturdays throughout
the year at the
Fair Haven Community Health Center, 374 Grand Avenue,
New Haven, CT 06513 phone 203-777-7411. Provides a place
for New Havens uninsured to receive primary care while obtaining
wellness education and access to social services.
Connecticut
WIC Program
is a special supplemental nutrition program that helps pregnant
women, new mothers and young children eat well and stay
healthy. Nutritious foods are given to those who qualify
at no cost to them. WIC helps women in need of better nutrition
who are pregnant, breastfeeding or just had a baby. WIC
also assists children and foster children under 5 years
old. Working families may also qualify. Call Toll Free in-state
at 1-800-741-2142 to ask for the nearest or most convenient
office. When you make an appointment you will find out which
form and documents you need to bring. WIC staff will confirm
if you qualify, and will review what you need to apply.
Hill
Health Center provides accessible, comprehensive and
quality primary and behavioral health care to low income
and undeserved residents in the Greater New Haven area.
City
of New Haven Health Dept. has a listing of local clinics
that offer immunizations at a low cost or for free.
The

Fair
Haven Community Health Center, provides Prenatal Care,
Comprehensive Primary Care, Wellness Center, Pediatric Care,
Social Services, On-Site Laboratory, Youth, Nutrition, and
Special Programs in the Greater New Haven area. The Center
is a participating provider in many health insurance plans,
including Medicare and Medicaid. For those without insurance,
payment is assessed on a sliding-fee scale. Funding from
Federal, State, City, and private sources helps the Center
maintain its fiscal viability while serving many patients

Naugatuck

203-924-9548 Immunization Clinics are held on the second
and last Tuesday of each month from 2:00 PM-3:30 PM by appointment
only. Immunizations are provided to all children that do
not have health insurance to cover vaccines. A child must
be accompanied by a parent or guardian and have a record
of all previous immunizations given. The charge for immunizations
is $5.00 per family for each visit. Call the Immunization

is a school Based Health Center at Hamden High, 203.248.6107.
It is a licensed outpatient clinic and provides a full array
services. Mom's Clinic is operated by the Quinnipiack Valley
Health District, and supported through a grant from the
State of Connecticut. Monday through Friday 8:00am- 2:00pm.
The Clinic is open during the school year except legal holidays
and vacations. (See After Hours Policy.) There are no fees
for services performed at the clinic. Fees are covered by
a grant from the CT Dept. of Public Health for all students,
with or without insurance.
Saint
Raphael's Healthcare System Smiles to Go The first of
its kind in Connecticut, Smiles to Go! is a mobile dental
clinic on wheels that travels to elementary schools and
other locations throughout the city to provide basic dental
care to those in need.The 40-foot clinic-on-wheels is staffed
by a pediatric dentist and hygienist, and features two examination/treatment
rooms for basic dental services, including routine cavity
repair. Studies have shown that good dental care is essential
for overall health and well-being. Lack of dental care can
lead to other serious health problems. For more information,

Saint
Raphael's Healthcare System Project Mothercare For more
than a decade, the award-winning Project MotherCare has
been traveling the streets of New Haven and surrounding
towns providing prenatal care to pregnant women, as well
as primary care. Services also include physicals and immunizations,
as well as nutritional and substance abuse counseling. The
clinic, housed in an 18-wheel tractor-trailer on wheels,
travels to different neighborhoods each day, and follow-up
referrals are made as necessary. Fees vary by service and

Saint
Raphael's Healthcare System Project HOPE Project HOPE
is an award-winning Saint Raphael's initiative that provides
training, education and counseling to women who are making
the transition from welfare to work. More than 100 women
have graduated from the program since its launch in 1997.
Participants receive classroom training and one-on-one mentoring
on topics such as interviewing, resume preparation, time
management and customer services, and are taught about workplace
diversity and meeting employer expectations. For more information,

Raphael's award-wining Project ElderCare has eight clinics
that provide primary care and social services to hundreds
of seniors in easy-to access community settings, including

Saint
Raphael's Parish Nurse Program reaches out to 32 community
churches and synagogues. The program consists of volunteer
parish nurses, who are each members of their specific church
or synagogue. The parish nurses work with and actively recruit
other volunteers to provide health information and community
referrals in their house of worship, as well as sponsor
screening and support groups for members of their congregation.
Most are registered nurses, and Saint Raphael's provides
an intensive nine-week training course that covers everything
from spiritual wellness to the importance of listening and
learning. The
program is designed to help eliminate confusion about healthcare
services and to improve access to health care. Services
might include blood pressure and cholesterol screenings,
diabetes and vision checks or flu shot clinics, and some
nurses follow up on members who have health problems or
were recently released from a hospital, to help ensure they
are getting services they need. For more information on

Saint
Raphael's WIC Program WIC is a federally funded program
that nourishes more than 11,000 women, 15,000 infants and
33,000 children each day in Connecticut. The programs goals
are to prevent health problems and improve nutrition. WIC
provides vouchers for specific foods--milk, cheeses, eggs,
fruit juice, dried beans and peas, peanut butter, and iron
fortified cereal and infant formula. Eligible participants
enroll in the program at their local WIC office, like the
one at Saint Raphael's, which serves more than 2,500 clients
in Greater New Haven. WIC
does more than just supply nutritious foods. It also provides
individual counseling and group classes. Mothers learn about
good nutrition during pregnancy, while breast-feeding, and
throughout early childhood. They also learn about the dangers
of substance abuse and the importance of health care and

Raphael's Sister
to Sister Program:
Since 1995, the Hospital of Saint Raphael has been a proud
participant in the Connecticut Breast and Cervical Cancer
Early Detection Program (CBCCEDP). This program is both
Federal and State funded and is offered at the Hospital
in conjunction with the Connecticut Department of Public
Health. The program provides free breast and cervical cancer
screening and diagnostic services. Eligibility criteria
to participate in the program are women, who are 40 and
older, uninsured or underinsured and have a low income.
Women who participate in this program may also be eligible
to participate in WISEWOMAN, a program offering free blood
pressure, cholesterol and diabetes screening. For further

We're
Special Too! is a free sibling support program for brothers
and sisters of children with special needs, such as hospitalization,
handicapping conditions, chronic or terminal illness. Sponsored
by the Friends of the Yale-New Haven Children's Hospital
and coordinated by a Certified Child Life Specialist, this
program encourages self-expression and develops trust in

New Haven exists to serve the New Haven residents of
the neighborhoods located in the federally designated Empowerment
Zone. We are doing that by supporting various agencies that
are focused on helping residents develop job skills, seek
out job opportunities and improve levels of education. In
addition, we support initiatives that help residents overcome
obstacles to obtaining employment, and factors that prevent
them from keeping employed. In addition, we are helping
residents improve their overall quality of life by adding
substantial financial support to homeownership offerings
available in the EZ neighborhoods.
