Your Appointment: What to Bring with You

The Yale Eye Center physicians ask that you bring the following items with you for your appointment:

• Your current eyeglasses and/or contact lenses
  
  
• List of all current medications (include the name of the medication and the dosage)
  
  
• Medical records that relate to your current condition. This should include X-rays, CT scans and any additional test results or copies of medical records.
  
  
• Your current health insurance identification card and any referral/authorization forms that may be required by your insurance company

We will do our best to see you promptly at your scheduled appointment time. Because of the nature of our practices, we sometimes have emergencies that can cause a delay. We appreciate your understanding when other patients require extra time, and we promise to give you the same undivided attention when you are seen.

Yale Eye Center Office Hours

The Yale Eye Center is open from 8:00 a.m. to 4:30 p.m. Monday through Friday. Emergency coverage is available as needed 24 hours a day. Call us at 203-785-2020 or toll-free at 1-800-395-7949 after hours to contact a Yale physician in an emergency.

What to Expect on the Day of Your Visit

The Yale Eye Center is part of a teaching hospital. During your appointment, you and your Yale physician may be joined by another physician who is an "ophthalmologist in training." Please know that this enhances the quality of care that you receive at Yale.
