Oculoplastics and Orbital Surgery is led by Dr. C. Robert Bernadino. Dr. Bernadino provides treatment of diseases affecting the eyelids, tear duct system, and orbit (tissue and bone surrounding the eye). Services include rejuvenation of the upper and lower eyelids, brow and forehead area using the latest CO-2 laser technology.

Your Appointment: What to Bring with You

The Oculoplastic Service physicians ask that you bring the following items with you for your appointment:

  • Your current eyeglasses and/or contact lenses

  • List of all current medications (include the name of the medication and the dosage)

  • Medical records that relate to your current condition. This should include X-rays, CT scans and any additional test results or copies of medical records

  • Your current health insurance identification card and any referral/authorization forms that may be required by your insurance company.
