Getting Here

Our physician practices are located on the 3rd floor of the Temple Medical Center at 40 Temple Street in New Haven, Connecticut. Patients may use valet parking services, the Temple Street Garage, connected directly by a second story skywalk, or the Temple Medical Garage, connected directly to 40 Temple Street on the first level. The cost of parking for either self-parking or valet parking is the same: up to 1 hour is $3.00, up to 2 hours is $5.00, up to 3 hours is $7.00. The maximum cost for parking is $9.00

Your Appointment: What to Bring with You

The Yale Eye Center physicians ask that you bring the following items with you for your appointment:

• Your current eyeglasses and/or contact lenses
• List of all current medications (include the name of the medication and the dosage)
• Medical records that relate to your condition. This should include x-rays, CT scans and any additional test results or copies of medical records.
• Your current health insurance identification card and any referral/authorization forms that may be required by your insurance company

We will do our best to see you promptly at your scheduled appointment time. Because of the nature of our practices, we sometimes have emergencies that can cause a delay. We appreciate your understanding when other patients require extra time, and we promise to give you the same undivided attention when you are seen.

What to Expect the Day of Your Visit

The Yale Eye Center is part of a teaching hospital. During your appointment, you and your Yale physician may be joined by another physician who is an "ophthalmologist in training." Please know that this enhances the quality of the care that you receive at Yale.

If You are Having Surgery

You and your ophthalmologist will discuss the benefits, risks and options for your surgery. You will be asked to sign a consent form, agreeing to surgery.  You will be required to have pre-op medical clearance by your primary care physician.

Where to Park

Yale New Haven Hospital:

1. Provides free valet parking for patients having 1-day surgery.

Temple Surgical Center: There are two garages connected to the Temple Medical Building (patients will be charged an hourly rate to park in either of these garages):

1. Temple Medical Garage
2. Temple Street Garage

What to Expect on the Day of Your Surgery

You will arrive at the surgical center approximately 1 hour prior to your surgery. After you have checked in, an O.R. nurse will bring you and in and prepare you for your procedure. Before your procedure, you will also have a chance to meet with the anesthesiologist and your ophthalmologist. After your surgery, you will stay in the recovery unit until it is determined by your ophthalmologist that you can leave. You will be given post-op instructions when you are discharged.
