REMINDER: Renewal requests must be received by the HIC 60 days before their expiration in order to avoid lapsing. Please be advised that it is a violation of Yale policy and federal regulations to continue research activities after the approval period has expired. If the HIC has not reviewed and reapproved this research by its current expiration date, all enrollment, research activities and interventions on previously enrolled subjects must stop. |
