The outcome of patients with acute renal failure remains unchanged in the last five decades despite tremendous advances in understanding the pathophysiology of acute renal failure at the cellular and molecular level. This issue is becoming increasingly recognized with an urgent need for patient oriented research to improve outcomes for this common condition.

My research focuses on determining optimal clinical strategies and identifying opportunities for improvement in the prevention, treatment, and outcome of acute renal failure. Specifically, I am studying the epidemiology of acute renal failure occurring with hematopoietic cell transplantation (bone marrow transplant). This human model of acute renal failure resembles the commonly seen renal dysfunction in the critically ill patients in the ICU. I am also investigating newer therapies for acute renal failure with compounds, such as Urodilatin, that are selective vasodilators. I am also involved with translating biomarkers such as urine IL-18, from bench to bedside, for early diagnosis of ARF. Delayed diagnosis of acute renal failure with traditional markers, like serum creatinine, is one of the main reasons for failure of interventional strategies in this setting.

In addition, I am building a consortium of transplant nephrologists to address medical, economic and psychosocial outcomes in kidney donors. The long-term outcomes in living kidney donors and the long term risks of donation are largely unknown. Our efforts will help us guide health policy and improve the informed consent process with living kidney donation.
