Medical Insurance Information

We participate in most area HMOs and all major health insurance plans. For some insurance plans, a referral form or referral approval from the primary care physician may be required. If the patient is not covered by insurance, we appreciate payment when service is provided.

The Yale Center for Sleep Medicine has three accredited sleep centers in New Haven, shoreline area (Guilford), and Norwich (New London County) for the convenience of our patients. Please call the one nearest you to schedule an appointment.
