  • Please use a separate form for each item requested (After completing a request, press the button to submit additional requests).
  • Students and non-Medical Center Yale affiliates are charged $6/chapter.
    [Note: This fee can be waived for students (studying, on rotation, or conducting research) away from the New Haven area.  Please contact your Personal Librarian to request this waiver]. 
  • A $6/chapter fee will also be charged to any Yale patrons requesting copies of materials available through Yale’s electronic resources. Prepayment is required. If you wish to prepay using a credit card, please fill out this form. If you are unable to fax it back, please bring it to the Administrative Office in the Cushing/Whitney Medical Library, SHM, 333 Cedar Street, room L110.  We are unable to take credit card information over the phone.
