Digital Camera Request Form

The Library now offers two Sony Mavica digital cameras for use of the members of the Yale-New Haven Medical Center community.

NOTE: By completing and submitting this form you are requesting to reserve the digital camera and agreeing to the terms of its use listed below. We will contact you to let you know if the camera is available during the period you wish to use it.

Circulation Policy and Terms of Use for the Digital Camera

  • Please use and handle the digital camera with care. An instruction pamphlet and quick reference guide are included with the camera.
  • The digital camera will only circulate to members of the Yale-New Haven Medical Center community with a valid Yale University, Yale-New Haven Hospital, or Medical Library ID.
  • The loan period is 72 hours.
  • Renewal
    • Unless previously reserved, you may renew the loan period, once, for an additional 72 hours.
    • Telephone renewals are not accepted.
    • The camera is subject to immediate recall after the initial loan period.
  • The late fee is $25.00.
  • IMPORTANT: The camera must be returned to the Circulation Desk where it was checked out and the camera must be received by a library staff member. Failure to do so may result in the possible cost of camera replacement.
  • Immediately report any damage or malfunction upon return of the camera.
