Medical Center Student Laptop Computer Request Form

The Library has 15 Dell Latitude laptop computers exclusively for Medical Center student use.

Software on the computers includes:

  • Windows XP Professional
  • Microsoft Office XP
  • EndNote X
  • Adobe Acrobat Reader 8.1.
 Student laptop.

Features include; built-in wireless connectivity, print capability on library printers, and a CD-RW/DVD-ROM Drive.

A laptop may be checked out of the Library for three days and will be available on a first-come/first serve basis through the Circulation Desk of the Medical Library unless reserved with this form.

Students will be required to show a valid Yale University ID and complete a Student Laptop-Borrower Responsibility Form (SLBRF) (PDF Document, Download Adobe® Reader® ) in order to check out a laptop. For your convenience you can read and complete this form now. Bring the completed SLBRF with you when you come to borrow the laptop. Once an SLBRF is completed and returned, it is kept on file in the Circulation Office of the Medical Library. You will only need to complete an SLBRF once.

At the time of Checkout and again at the time of Return, the Student and a Library Staff member will complete a short checklist of each item provided for the loan and their current visual condition. This is to ensure all components are present and/or accounted for.
