Since the pediatric patient population represents the majority of RSV admissions, the risk for nosocomial transmission is greatest in this group of patients. To prevent nosocomial transmission of RSV in the pediatric inpatient population, the following 3 Level approach (in addition to the control measures delineated above) is to be used during the respiratory virus season.

Level 1:
Level 1 measures are to be instituted when there are four or more RSV admissions in any 14-day period.
Screen symptomatic patients for RSV by using rapid respiratory antigen panel. If the rapid RSV diagnostic test result is not available on admission, place the patient on both Contact and Droplet Precautions until the test result is known.
An informational pamphlet should be given to patients' families explaining the need for these precautions.
Limit number of visitors to two at a time per patient.
Screen and exclude visitors for signs and symptoms of respiratory infections. Screening should be done by Business Associates and Nursing Staff.
Instruct family members/visitors to wash hands when coming and going from rooms.
Instruct family members/visitors with RSV-infected children not to visit other hospitalized children or common areas within the hospital.
If the rapid RSV diagnostic test result is not available on admission, place the patient on both Contact and Droplet Precautions until the test result is known.
Place appropriate signage at the entrance of all pediatric floors instructing visitors with respiratory symptoms not to visit.
Equipment such as pulse oximeters and electronic thermometers should be cohorted with patients who have RSV.
