Discharge to Home
Discharge of a VRE colonized/infected patient to home is generally not of a concern as this organism does not colonize healthy individuals. However, if there are immunocompromised persons in the household, this should be discussed prior to discharge.

Discharge/Transfer to Another Medical Facility or Home Health Care Services
If the patient is discharged to another hospital, ECF, or needs home healthcare services, the receiving facility must be notified in advance so that appropriate Isolation Precautions can be undertaken. The need for Contact Precautions for VRE should be indicated on the W-10 form at the time of discharge.
